Description
The Franciscan Healthy Living Center is offering A Healthier You!, a proven lifestyle change program to improve health and delay or prevent Type 2 Diabetes. A trained lifestyle coach guides and encourages a small group during this program. A Healthier You! is a year-long program that begins on Aug. 15 and is held from 10 to 11 a.m. (EST). The small group meets weekly for 6 months and then every 2 weeks for 6 months. --- Program Details: -Follows CDC curriculum with proven results to help individuals lose weight and lead a healthy lifestyle -Sessions begin during the year with different times and locations -The next session will begin on Nov. 12 and will be held from 3:30 to 4:30 p.m. (EST) -Covered by some insurance plans (including Medicare) -Covered benefit for Franciscan employees by Employee Health and Wellness --- Eligibility -You may be eligible for this program if you are: -Overweight -Have a family history of diabetes -Have a history of gestational diabetes and/or -Have been diagnosed with pre-diabetes --- The sessions are held at the Franciscan Healthy Living Center at 1116 N. 16th Street Lafayette, IN 47904.
